Summary

This document provides a structured overview of the main health problems recorded since September 2025.

The purpose is to preserve the chronology, new symptoms, new clinical findings, and major management decisions while keeping detailed measurements, treatment schedules, and specialist-level observations in separate supporting records.

No single shared cause is assumed.

Background

Different parts of this history have been assessed by GP, FCP, physiotherapy, Musculoskeletal Services, Urgent Treatment Centre, and other clinical services.

Several symptom groups appeared within relatively close time windows. These overlaps are recorded because they may be clinically relevant, but the document does not assume that symptoms occurring close together have the same cause.

Details

Phase 1 – September 2025 to early February 2026: initial lower-limb problem, weight reduction, and bilateral calf heaviness

Phase 2 – February 2026: acute low-back pain, right-hand wound, urgent assessment, and subsequent treatment

A later medicine-reaction concern developed only after treatment had begun and is recorded in Phase 3.

Phase 3 – late February to early July 2026: suspected medicine reaction and recurrent right-hand skin cycle

Phase 4 – July 2026: community MSK review followed by clustered skin, systemic, and right upper-limb changes

The timing of the July events is preserved because several symptom groups became active within a relatively short period. No causal relationship between the MSK exercise programme, skin changes, systemic symptoms, or right upper-limb problem is assumed.
