Subject: Request for Referral to Specialist Clinic and Inquiry on Data Collection for DSD / Intersex Assessment

I hope this message finds you well. I am writing to formally request a referral to a specialist clinic for the assessment of a possible Disorder of Sexual Development (DSD) / Intersex condition. This referral is sought for personal health management and for documentation purposes in relation to administrative or government requirements.

For context, the following assessments have already been conducted under your supervision:

• Personal medical history and self-reflection on relevant symptoms • Blood tests and hormone profiling (sex hormones, LH/FSH, estradiol/testosterone) • Initial clinical observations conducted at GP level

In addition, I have observed the following, which may be relevant to the DSD / Intersex condition:

  1. Dental Development:

My dentition pattern is currently 2122, which includes a missing molar and an atypical arrangement of the second molar in a position typically occupied by the third molar (wisdom tooth).

  1. Sexual Development:

I suspect there may be an abnormality in my sexual development, particularly with secondary sexual characteristics. I would like to request endocrine testing (e.g., testosterone, estradiol) to evaluate any hormonal imbalances or genetic testing to check for potential chromosomal anomalies.

  1. Learning Difficulties:

I suspect I may have learning difficulties, particularly related to reading and language processing. I would appreciate a comprehensive learning assessment to determine whether there is a cognitive or developmental component involved.

I would appreciate your assistance in confirming whether the following research teams are currently seeking adult cases for data collection on DSD / Intersex conditions:

• Cambridge Clinical Research Facility (CRF), Addenbrooke's Hospital – Adult DSD Clinic / Clinical Research Program • UCLH / NIHR BioResource – Intersex/DSD Cohort, genetic and longitudinal cohort research

As many DSD / Intersex cases typically involve younger individuals with prior medical intervention, adult cases without previous intervention are relatively rare. Since I have not yet started hormone replacement therapy (HRT), I can still provide original data related to my condition. However, if I proceed with HRT, I will no longer be able to provide this baseline data.

If these teams are interested in including my case for research purposes, I would be happy to assist with any necessary information or assessments. If the teams do not require additional data, I will proceed with my ongoing healthcare plan and start HRT.

I understand that these teams may focus on rare adult cases with no prior medical intervention. If my case aligns with their criteria, I would be happy to proceed with the next steps.

If the teams do not require additional data, I would also like to understand how to obtain an official diagnostic assessment for personal or administrative purposes, either through a specialist clinic or another pathway.

Additionally, I would appreciate a referral to an appropriate multidisciplinary team for a comprehensive evaluation, if deemed necessary by you. If hormone replacement therapy (HRT) is to be started and you believe a referral is not required, I am happy to proceed with my ongoing healthcare plan.

The specialist assessment would provide:
