Subject: Documentation of New Symptoms and Symptom History (Hand Wound) Dear Dr
I am writing to document the recent symptoms related to the wound on my right hand, in conjunction with the timeline provided in my previous document regarding my other symptoms. This will ensure a complete and consistent record for my upcoming referral and future consultations.
- 22 February 2026 (Sunday night at 10:00 PM) - Hand Itching and Wound
At the same time as my back pain, I also had a wound on my right hand. The wound on my right hand (on the back of my hand) was itchy and uncomfortable, causing significant irritation and distraction from my back pain. I could not focus fully on the back pain due to the irritation from the hand wound. The wound caused continuous discomfort throughout the evening, making it hard to sleep.
- 23 February 2026 - Persistent Hand Wound Symptoms and Need for Immediate Treatment
On the morning of 23 February, the itching and discomfort from the hand wound persisted. The wound on my right hand (on the back of my hand) became increasingly sensitive and irritated, further affecting my ability to focus on other issues. The pain from both my lower back and hand wound made it very difficult to manage. As I was in severe discomfort, I went to the UTC (Urgent Treatment Centre) seeking immediate treatment for both the pain and itching. The irritation from the hand wound and the pain from my back were both urgent issues that needed to be addressed, which led me to seek relief at UTC instead of waiting for a GP appointment. The symptoms continued throughout the day, and the discomfort from both issues was quite overwhelming.
Sincerely,
